Specimens may be taken separately from each side of the nose for nasal secretions. Secretion is most easily and readily collected by having the patient blow the nose on a waxed paper or cellophane handkerchief. This gives a specimen which represents only nasal secretion or a collection of both nasal and sinus secretions. Small crusts from the septum and vestibule of the nose should be avoided. If no secretion is available from blowing the nose, it may be necessary to remove it by swabbing it with a cotton applicator or postnasal secretions may be obtained through a suction cannula. Secretion may also be collected in a specimen bottle at the time of acute exacerbation. The specimen should then be smeared on a clean microscope slide and covered with another clean slide with toothpicks in between. |

Many toilet sanitizers generate chlorine and may cause false-positive reactions to guaiac tests. False positives can be traced to toilet bowl water containing blood from menstruation or urine. Toilet sanitizers and detergents may reduce hemoglobin concentration or antigenicity of immunologic-based tests. |

If multiple tests are being drawn, draw coagulation studies last. If only a fibrinogen is being drawn, draw 1-2 mL into another Vacutainer, discard, and then collect the fibrinogen tube. The collection procedure avoids contamination of the specimen with tissue thromboplastin. When it is necessary to obtain blood samples from critically ill patients from indwelling arterial or venous catheters, the heparinized fluid infusion should be stopped and the first 15 mL blood obtained through the indwelling catheter should not be utilized for any coagulation studies. The appropriate volume of blood for the needed coagulation studies should be withdrawn from the catheter and the heparin infusion resumed after obtaining the sample. Samples obtained from a catheter should be so indicated. It should be understood that all coagulation specimens should be obtained without heparin contamination. All coagulation tests are, to some extent, sensitive to heparin contamination. |
